Proof of Claim Form

[07.16.08] – As previously instructed in the www.afaaloha.com website posting dated and titled:

 [03.31.08] – Bankruptcy Documentation and Instructions,

A  Proof of Claim form was sent to all Aloha Airlines employees needing to file a claim with the Bankruptcy court for any outstanding values owed to you from the company. The original deadline for filing the claim was July 22. Due to the conversion from Chapter 11 to Chapter 7, the new deadline for filing the claim is now August 18.

Such claims would typically include earned accruals, such as sick leave, vacation, and occupational banks. As stated in the above referenced posting, AFA has compiled a group claim on your behalf which includes all accrued bank balances as of March 31, 2008. It is NOT necessary for you to file an individual claim unless you are owed monies by the company other than what is listed above. Please refer to the above referenced posting for further information.
